A comprehensive virtual evaluation with your licensed Ontario Nurse Practitioner. Includes a thorough platformal interview covering your history and current symptoms, the platformal standard ADHD rating scales, review of your medical and developmental background, and more. A formal written assessment report is delivered within two weeks of your appointment.
Adults 18+ ยท New clients welcomeOnce your report is complete, you are welcome to book a focused appointment to review your results with the provider. This is where you discuss your diagnosis, understand the findings, and — if the required results have been submitted beforehand — explore medication options as a part of your personalized treatment plan.
Existing clients onlyIf medication has been determined to be a safe and appropriate part of your care, you may choose to continue with Calebra for ongoing medication management or return to your primary care provider. If you choose to continue with Calebra, regular follow-up appointments will keep your treatment optimized. Progress reviews, prescription renewals, dosage adjustments — all virtual, all on your schedule. Frequency varies from every two weeks to every three months depending on stability.

While recommended accommodations are included in your assessment report, this appointment is for clients who require a very specific or specialized letter — for example, when an employer or school has requested a particular form or format not covered by the standard report.
Official signed letter confirming your ADHD diagnosis and prescribed psychostimulant medication โ so you can travel domestically or internationally without issues at airports or customs.
